Hand in Hand Options Program Application for Admission Procedures
The following items are required to complete the admissions process for the Hand in Hand Options Program at Pope John Paul II High School:
**1. Hand in Hand Options Program Application for Admission —Applicant must complete an application.
**2. Application Fee—The $60.00 fee must be submitted with the application and can be paid online.
**3. Report Card—A copy of the applicant’s previous year-end report and most recent report card are required.
**4. Individualized Education Plan (IEP)—A copy of the applicant’s most recent IEP is required.
**5. Psychological Evaluation Report and Eligibility Report—A copy of the applicant’s most recent psychological evaluation report(s) and eligibility report are required.
6. Teacher Recommendations—Two recommendations are required. One recommendation should come from the applicant’s current special education teacher. The other recommendation should come from one of the applicant’s current general education teachers. Each teacher should complete the Recommendation Form.
**7. Essay—Each applicant is required to write an essay in his/her own handwriting following the guidelines on the form.
8. Parent/Student Interview and Campus Visit—Applicants will be scheduled for an interview with the Admissions Office and Hand in Hand Options Program Coordinator.
9. Student Day Visit—An appointment to visit JPII for the day will be coordinated with the Hand in Hand Options Program Coordinator following the parent/student interview.
**10. Immunization Record—A current immunization record is required for admission. Please provide a copy of the applicant’s most recent immunization record. The State of Tennessee requires the following vaccinations: 5 DTP, 3 Polio, Hepatitis B series (2 or 3), and 3 MMR.
11. Medical Examination Form—A medical examination is required for admission. The medical examination must be dated after May 1. Details regarding medical examinations will be provided in the spring.
